Show where files come from on File page

When I go to the Files library, I can click on files that were added via Plan/Incident/etc, but that information is now shown. Above the "inherit permissions" checkbox, we should have a field for "Parent Document" that links back to it.

Allow easier differentiation between users or internal contacts with the same name.

When selecting a user or an internal contact by name, if a number of people share that name, it's not clear which one to select. This needs to be clearer.

Ability to restrict users from changing approvers and editors. There should be an option in the user roles to remove the ability to edit (ie add/remove) approvers and editors and only select administrative users should have this ability.
