OSHA 301 Log Additions to Claim Object

On the OSHA 301 log that we would like to create as a new standard report we need to have 2 fields added to the Claim__c Object in order to do this.

Physician_Name__c

Physician_Details__c

We can then utilize the fields in the standard package and not have to have them created each time
