HC Event Reporting - Display patient fields for Near Miss or Unsafe Condition within portal

Near Misses and Unsafe Conditions may pertain to a patient and that data needs to be captured during the event entry. The enhancement needs to be across all event reporting portals. Possibly adding a question if a patient was involved and the rendering the patient fields
